The Falls and Rehab team is a group health professionals who work together to check all the things that might affect your risk of falling.
- a trained professional will check all the different things that might affect your health during a clinic visit, or at your home if you can’t get out
- gives help and advice to other health workers
- is the main place where people are sent if they have had a fall
- checks that the falls procedure is being used properly and that people are sent to the hospital when needed
- carries out detailed checks to understand someone’s risk of falling or getting osteoporosis
- works to prevent falls across Warrington
- we work closely with the falls consultant, A&E, and the hospital
- helps teach staff about preventing falls, osteoporosis, and how to use the first assessment tools
Who can access the service?
Adult over 18 residing in Warrington with a Warrington GP.
Referrals will be accepted from:
- GP and consultant
- health care professionals (AHP’s, nurses & paramedics)
- social care professionals. (O.T & Social Workers)
- nursing and residential care homes via the Enhanced Care Home Support Team
Where is the service offered?
We are community based and visit patients in their homes or residential / nursing/ care homes.
