The Halton Wellbeing Nurses team provides comprehensive physical and mental health reviews delivered by registered nurses across Halton.
Who we are
We are a team of experienced mental health and general nurses based at The Bridges Learning Centre, Widnes. Working closely with all GP practices in Widnes and Runcorn, we offer annual physical health checks tailored to your individual needs.
Who we support
- Are registered with a GP practice in Widnes or Runcorn
- Are 18 years or older
- Are currently on the Severe Mental Illness (SMI) register
Our services
- Annual Health Checks – blood pressure, BMI, and overall health review
- Lifestyle Advice – nutrition, exercise, and wellbeing guidance
- Referrals – access to smoking cessation, alcohol/drug support, and specialist services
- Blood Tests – cholesterol, diabetes, and other key health markers
- Additional Tests – tailored checks depending on your needs
- Medication Support – discussions about your medicines and answers to any questions
How to access our services
Referrals are made directly by your GP. If you are eligible, your GP will send your details to us.
Once we receive your referral, a member of our team will contact you to arrange an appointment.
Appointment information
- How we contact you – We will phone or write to you to arrange a clinic appointment or home visit. Please let us know your preferred method of contact.
- Cancelling appointments – If you need to cancel or change your appointment, call us on 0151 511 5694 or contact your GP practice as soon as possible. We’ll help you rebook at a convenient time.
- Home visits – If attending the clinic isn’t suitable, we can arrange a home visit. Please mention this when we contact you.
