Referrals are accepted via medical, health or educational professionals via a referral form.
Physiotherapy and Occupational Therapy referrals should be sent to: bchft.haltonchildrenstherapy@nhs.net.
All referrals to be submitted using our single referral form for Children’s Specialist Services in Halton indicating which service is required.
Referrals for Physiotherapy must be accompanied by Appendix 2.
Referrals for children with motor skill difficulties to Occupational Therapy (with or without Physiotherapy referral) must also be accompanied by the teacher and parent/carer questionnaires for school aged children or teacher and parent/carer questionnaires for pre-school age children.
Please ensure detailed information is submitted to ensure a clear picture of the child’s difficulties can be considered when triaging. Insufficient information may result in the referral not being accepted.
Interpreters are available – please contact the service to arrange.
Referral criteria
Referrals considered from:
- Children with Halton GP
- Children with Frodsham/Helsby GP
- Children attending special school in Halton
