Intermediate Care – Warrington

Intermediate care is a range of needs led, transitional and integrated services that are intended to improve patient health outcomes:

  • To promote faster recovery from illness
  • To prevent unnecessary presentation and admission to an acute hospital bed
  • To prevent premature or unnecessary admission to long-term residential care
  • To maximise independent living reducing the need for long-term care support

These services are delivered in partnership between community health care and the local authority and include:

  • Therapy lead rehabilitation
  • Personal care and social support
  • Nursing support
  • Medical support

Ther service works with people in a range of setting, including at home or in designated care environment.

Intermediate care includes a programme of active rehabilitation involving an appropriate combination of occupational therapy, physiotherapy and medical/nursing input.

Intermediate care services work with people to support them to regain as much independence as possible. The service always aims to support people to stay at or return home wherever possible.
