The Macmillan Palliative Care Team is a community team of Clinical Nurse Specialists with the Knowledge and skills to provide specialist advice and support to patients and their families with life limiting illness.
This includes patients who have diseases other than cancer.
This is a Specialist Service encompassing complex pain and symptom control, psychological support and emotional support, offering advice to GPs and District Nurses and sign posting to other services where appropriate.
The Clinical Nurse Specialists work very closely with patients and their family, carers and other health care professionals within the multidisciplinary team as well as with other specialist palliative care providers in the Hospice and Hospital.
The team is supported by a Consultant in Palliative Medicine.
Who can access the service?
The service is for adults over the age of 18 years and with a life limiting illnesses who agree to being contacted by the Specialist Team.
Referrals can be made by any health or social care profession working with people with life limiting illnesses.
The team accept self-referrals from patients or their family, however further information will be sought from the patient’s GP.
Where is the service offered?
The service is offered in the patient own home.
