Community Neurorehabilitation Service – Halton

Halton Community Neurorehabilitation Service is a community based team of highly specialised health professionals with the knowledge and skills to provide assessment, and rehabilitation for people with neurological conditions and their families.

This can include Acquired Brain Injuries, Stroke, MS, Parkinson’s disease and other neurological conditions.

The core team consists of Neuropsychology, Occupational Therapy and Physiotherapy.

We work in a multidisciplinary way, which means that the team meet up regularly and liaise with other agencies involved to make sure that we are working towards a shared therapeutic goal.

The focus of the service is to improve the quality of life for people living with the effects of long term neurological conditions, by supporting them to achieve their optimal cognitive, physical, behavioural, and emotional levels of functioning with the goal of maximizing their re-integration into the community.


Who can access the Neurological Rehabilitation service?

  • 16 years old and above
  • Confirmed neurological diagnosis
  • Registered with Halton GP

How can people access the service?

Referrals can be made by any health or social care professional.


Where is the service offered?

The Halton Community Rehabilitation Service is based at the Independent Living Centre in Runcorn.

Patients are offered appointments either at a clinic base, at home, or in care/residential homes, based on clinical need
