You are entitled to access and receive copies of the health records that Bridgewater Community Healthcare NHS Foundation Trust holds about you.
In accordance with the Data Protection Act 2018 patients are entitled to copies of their information by making a ‘Subject Access Request’.
You may also have a right, under the Access to Health Records Act 1990 to access information about others.
The procedure for requesting access on behalf of someone else’s health records is as follows:
- Accessing records for children and individuals who are deceased, the application on behalf of someone else form; needs to be completed.
- A request is made to the areas that hold the relevant records, please find a list of services within the Trust and direct your application to them. If you cannot find the services on the list please direct to Trust headquarters (address below).
