ASD/ADHD school report Template

The team may require more up to date information from schools about specific difficulties a child/young person may be experiencing in school.

We may ask schools to provide us with these reports at any point of the assessment.

 

CAST questionnaire

The Childhood Autism Spectrum Test, abbreviated as CAST, is a tool to screen for autism spectrum disorder in children aged 4–11 years, in a non-clinical setting.

 

SNAP questionnaires

These questionnaires are tools for parents and teachers to screen for symptoms of ADHD.
