Community Specialist Palliative Care – Halton

The Community Specialist Palliative Care is a community based team of Clinical Nurse Specialists with the knowledge and skills to provide specialist palliative advice and support to patients with life limiting illnesses.

This is a Specialist Advice Service encompassing complex pain and symptom control, psychological support and emotional support, there is also the facility to sign post to other services.

The Clinical Nurse Specialists work very closely with patients and their families, carers and other providers of care (also known as Multi-disciplinary Team) including:

  • Integrated Community Nursing Teams
  • General Practitioners
  • Acute Oncology
  • Consultants
  • Outpatient and Day Care Departments
  • Family Support
  • Other Clinical Nurse Specialists
  • Pharmacists
  • Social Services/Voluntary Service
  • Physiotherapists/Occupational Therapists

Who can access the service?

Adults who are over 18 years of age, registered with a Halton GP and have a palliative diagnosis.


How can people access the service?

Using our referral form below referrals can be made by any health or social care professionals working with people with life limiting illnesses.

Self referrals can also be made by patients, their families and carers by telephoning the office.


Where is the service offered?

The service is offered in the patient’s own home or within our clinic at the Independent Living Centre.
